This conviction led her to establish the Chilkov Clinic in Santa Monica, where she provided direct, personalized care to countless cancer patients. As her practice grew and her impact widened, she recognized the need to disseminate her knowledge more broadly. This ambition culminated in the publication of her book, 32 Ways to OutSmart Cancer: Create a Body Where Cancer Cannot Thrive, and the creation of the OutSmart Cancer® website. In the year preceding her death, she faced a significant personal challenge when the devastating Southern California wildfires of January 2025 forced her to evacuate her home in Topanga Canyon. For months, she navigated displacement and uncertainty. On January 8, 2025, as flames neared her home, she shared her thoughts on Facebook, reflecting a remarkable equanimity: "My heart/mind is now shifting to pondering what it is I want to do, where and how I wish to live if all I own is turned to ash? I feel strangely calm. I am grateful for my many decades of meditation practice as I can find my equanimity as I face impermanence. At the same time, I am grieving for what has already been lost. It is so massive, so surreal, so apocalyptic…I cannot be attached. I left with a few articles of clothing and my hard drives and left everything else behind. My keyword for many years has been RESILIENCE…so now I am prepared to LIVE IT!"

Miraculously, her home survived the fires. Upon returning in the late spring of 2025, she shared her profound sense of gratitude and wonder: "Sitting at my house where I can SEE the hillsides where the fire came so close. 1/4-1/2 mile across the canyon and knowing that the fire also came to Tuna Canyon and Saddlepeak Road just one mile up the road…That is CLOSE!! And yet my neighborhood…this little thumbprint of Lower Topanga, though surrounded by fire, somehow survived and looks unscathed…And here I sit, gazing out at my magnificent wide view with my breath going in and out and my heart beating…and life goes on…Amazing. What a blessing to be here and whole."

Tragically, Nalini Chilkov passed away a little over nine months after returning to her beloved home, a poignant juxtaposition of her profound resilience and her eventual passing.
