These markers include body fat composition, blood glucose levels, cholesterol profiles, and systemic blood pressure. "However, there is increasing evidence that sleep health is about more than the number of hours you sleep each night."

The newly published framework identifies six core dimensions that compose holistic sleep health: duration (the total amount of sleep obtained per 24-hour period), continuity or efficiency (the ease of falling asleep and staying asleep without prolonged awakenings), timing (the placement of sleep within the 24-hour circadian cycle), satisfaction or quality (the subjective, self-reported contentment an individual feels regarding their rest), regularity (the consistency of sleep and wake schedules from day to day, including weekends), and daytime functioning (the degree of alertness, wakefulness, and cognitive performance experienced during waking hours).

The physiological pathways linking poor multidimensional sleep health to cardiovascular disease are intricate and profound. When sleep is irregular, frequently interrupted, or mistimed, the body’s autonomic nervous system remains in a heightened state of sympathetic activation. "It’s important to know that every individual has different sleep experiences, and these differences may contribute to other health inequities," Dr. St-Onge noted. "Including different components of sleep in discussions with patients provides essential information that can help health care professionals improve care."

The Technological Frontier and Life’s Essential 8

As consumer interest in health tracking reaches unprecedented levels, millions of individuals now rely on smartwatches, fitness bands, and ring-based monitors to quantify their nightly rest. While these commercial devices have successfully raised public awareness about the importance of sleep, the AHA scientific statement points out a significant scientific limitation: while current consumer technology excels at tracking sleep duration and estimating basic continuity through movement, measuring other critical dimensions—such as sleep timing regularity, architecture depth, and subjective satisfaction—remains challenging outside of a formal laboratory setting.

To build reliable clinical guidance, researchers emphasize the necessity of combining subjective, self-reported patient questionnaires with objective, validated physiological measurements.

Presently, sleep is codified as one of the core metrics within Life’s Essential 8, the American Heart Association’s landmark framework for optimizing cardiovascular health. However, within the current Life’s Essential 8 scoring algorithm, sleep is measured exclusively by duration (number of hours per night). This limitation exists purely because validated, standardized scoring rubrics for other multidimensional sleep components are still under development.

The publication of this scientific statement serves as a vital call to action for the scientific community to conduct rigorous clinical trials. While observational studies have overwhelmingly demonstrated that poor multidimensional sleep contributes to adverse cardiovascular events, there remains a pressing need for interventional trials proving that clinically modifying and improving specific components of sleep directly translates to enhanced cardiometabolic health outcomes.
